3. All entrants must apply by October 31st $1,000 Southwest gift card and a $500 Visa gift card by October 31st, 2017.
4. Only new AMN clinicians eligible to participate in this promotion. The winner will be selected through a random drawing of all participants and there will be one winner per brand.
5. The promotion runs from June 23rd, 2017 to October 31st, 2017.
6. The winner agrees that AMN Healthcare, Inc. and its affiliates, partners, directors, officers and employees shall have no liability in connection with the giveaway, participation in the giveaway and/or acceptance or use of the giveaway offered. By accepting, giveaway recipients agree to hold harmless American Mobile Healthcare and/or AMN Healthcare, Inc. for damages, injuries or losses that may be sustained in connection with the use or misuse of the giveaway. By participating in this offer, entrants agree to abide by and to be bound by these official rules.
7. The Visa and Southwest gift card is redeemable per the vendor's terms and conditions.
8. All decisions relating to entries, rules and/or selection of the giveaway recipient made by AMN Healthcare, Inc. is final.
9. Any federal, state and local taxes are the sole responsibility of winner. For current employees, the full market value of the prize will be included in their next paycheck and taxes will be withheld on their next paycheck. For non-employees, the full market value of the prize will be included on a Form 1099. The federal and or state taxes are their responsibility and they should put aside the appropriate amount of federal and state taxes to account for this liability. A form W-9 will be requested to obtain the relevant taxpayer information for purposes of filling out the Form 1099.
10. AMN Healthcare, Inc. is not responsible for late, lost, misaddressed or misdirected entries.
11. AMN Healthcare, Inc. reserves the right to change the rules without prior notice. All selections are final.
12. If the winner does not respond within 30 days of their prize notification or does not provide the required contact information could be disqualified and the prize award given to the next winner.
